consent button on American server

I just moved a survey and experiment from the server in Europe to the American server. The only glitch is that the section "confirm that you want to do this survey" does not appear at the bottom of the initial page. All I see is a big yellow "click this button to start survey." Has anyone else experienced this glitch? Any idea what to do? The IRB on my campus will not approve a study with a simple "click this button to start survey" -- they are very particular about the consent language and check box options.
